Information collected through Renovare&#8217;s public website that is not protected health information is governed by Renovare&#8217;s separate Website Privacy Policy, which describes how such information is collected, used, disclosed, retained, and protected. The Website Privacy Policy is available on Renovare&#8217;s website. Nothing in the Website Privacy Policy limits or replaces Renovare&#8217;s obligations under this Notice or applicable law with respect to protected health information.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">Information submitted through the Renovare website may be used to respond to inquiries, schedule services, communicate with prospective or existing patients, and provide requested services. communications maintained by Renovare are subject to additional confidentiality protections under Illinois law, including additional requirements concerning consent, subpoenas, and court orders. authority for disclosures that HIPAA alone might otherwise permit.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When Illinois law provides greater privacy protection than HIPAA, Renovare will follow definition of psychotherapy notes, we generally will obtain your written authorization before using or disclosing those notes except when HIPAA permits use or disclosure without Guardians<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Privacy rights concerning children and adolescents depend on:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The patient&#8217;s age<\/li>\n\n\n\n<li>Who consented to treatment<\/li>\n\n\n\n<li>The type of healthcare provided<\/li>\n\n\n\n<li>Illinois law<\/li>\n\n\n\n<li>Federal law<\/li>\n\n\n\n<li>Court orders or custody arrangements<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Parents and legal guardians generally have certain rights involving their child&#8217;s health information, but there are circumstances in which a minor may have independent confidentiality rights.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Behavioral-health records of minors can be subject to special Illinois confidentiality requirements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">Renovare may request custody, guardianship, or other legal documentation before releasing information.<\/p>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>Caregivers and Personal Representatives<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare recognizes that family members and caregivers may be important participants in a patient&#8217;s care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When appropriate, we may ask you to identify people with whom Renovare may communicate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A caregiver does not automatically have unrestricted access to your medical records.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We will consider:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Your permission<\/li>\n\n\n\n<li>The caregiver&#8217;s role<\/li>\n\n\n\n<li>Legal authority<\/li>\n\n\n\n<li>Applicable federal and Illinois law<\/li>\n\n\n\n<li>Your safety<\/li>\n\n\n\n<li style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">The minimum information reasonably necessary for the purpose<\/li>\n<\/ul>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>Telehealth<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare provides eligible healthcare services through telehealth.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Protected health information created during a telehealth appointment is subject to the same applicable privacy protections as information created during an in-person encounter.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We may use approved technology vendors and other business associates to provide:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Video appointments<\/li>\n\n\n\n<li>Electronic communication<\/li>\n\n\n\n<li>Scheduling<\/li>\n\n\n\n<li>Patient intake<\/li>\n\n\n\n<li>Electronic health records<\/li>\n\n\n\n<li>Prescribing<\/li>\n\n\n\n<li>Other healthcare functions<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Business associates that create, receive, maintain, or transmit protected health information on Renovare&#8217;s behalf are required to protect that information as required by applicable law and contract.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">Patients should participate in telehealth from a reasonably private location whenever possible.<\/p>\n\n\n\n<h1 class=\"wp-block-heading\"><strong>Home Visits<\/strong><\/h1>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare may provide eligible medical or behavioral-health services in a patient&#8217;s residence.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">During a home visit, other people may be present in or near the residence.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You should tell the provider if:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You would like another person to participate<\/li>\n\n\n\n<li>You would like to speak privately<\/li>\n\n\n\n<li>Someone present should not receive health information<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">We will make reasonable efforts to protect your privacy while providing care in the home environment.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Electronic Health Records and Patient Technology<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare may maintain health information electronically and may use electronic systems for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Medical records<\/li>\n\n\n\n<li>Scheduling<\/li>\n\n\n\n<li>Prescribing<\/li>\n\n\n\n<li>Patient intake<\/li>\n\n\n\n<li>Secure messaging<\/li>\n\n\n\n<li>Telehealth<\/li>\n\n\n\n<li>Insurance verification<\/li>\n\n\n\n<li>Billing<\/li>\n\n\n\n<li>Clinical decision support<\/li>\n\n\n\n<li>Other healthcare operations<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Access to these systems is limited according to job responsibilities and applicable privacy and security requirements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">When Renovare offers a patient portal or other secure electronic access method, patients may use it to access eligible health information and communicate with the practice according to the system&#8217;s capabilities.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Electronic Communication<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare may communicate with you by:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Telephone<\/li>\n\n\n\n<li>Voicemail<\/li>\n\n\n\n<li>Secure patient messaging<\/li>\n\n\n\n<li>Email<\/li>\n\n\n\n<li>Text message<\/li>\n\n\n\n<li>Mail<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">depending on your preferences, our available systems, and applicable law.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You may request reasonable alternative or confidential communication methods. Renovare may use third-party service providers to facilitate appointment reminders, scheduling communications and other administrative messages. Where required by law, Renovare will obtain appropriate authorization or consent before sending certain communications. Ordinary email and text messaging may carry privacy risks.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">Appointment reminders and administrative messages will generally contain only the information reasonably necessary for their purpose.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Business Associates<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare may use outside companies or professionals to perform services on our behalf.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Examples may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Electronic health record vendors<\/li>\n\n\n\n<li>Billing companies<\/li>\n\n\n\n<li>Telehealth vendors<\/li>\n\n\n\n<li>Secure communication vendors<\/li>\n\n\n\n<li>Information-technology providers<\/li>\n\n\n\n<li>Document storage companies<\/li>\n\n\n\n<li>Accounting or professional consultants<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">When these organizations handle protected health information on Renovare&#8217;s behalf and qualify as business associates under HIPAA, we require appropriate contractual protections.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Marketing<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare will not use or disclose your protected health information for marketing when HIPAA requires your written authorization unless you provide that authorization.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">General communications about:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Renovare&#8217;s services<\/li>\n\n\n\n<li>Treatment alternatives<\/li>\n\n\n\n<li>Care coordination<\/li>\n\n\n\n<li>Health education<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">may not constitute marketing under HIPAA in certain circumstances.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">We will comply with applicable law.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Sale of Protected Health Information<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare does not sell protected health information.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"margin-bottom:var(--wp--preset--spacing--bbe-50)\">If a proposed disclosure would constitute a sale of protected health information under HIPAA, we will obtain the authorization required by law before making the disclosure.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Fundraising<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare does not currently use protected health information to conduct fundraising campaigns.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If this practice changes, Renovare will comply with applicable notice, authorization, and opt-out apply.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Our Responsibilities<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Renovare is required by law to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Maintain the privacy and security of your protected health information<\/li>\n\n\n\n<li>Follow applicable federal and state privacy laws<\/li>\n\n\n\n<li>Follow the duties and privacy practices described in the Notice currently in effect<\/li>\n\n\n\n<li>Provide you with a copy of this Notice<\/li>\n\n\n\n<li>Notify affected individuals when required if a breach occurs that may have compromised the privacy or security of protected health information<\/li>\n\n\n\n<li>Provide additional privacy protections when federal or Illinois law requires
