Who it’s for
When to consider home visit
A primary care home visit may be helpful when health-related circumstances make traveling to a medical office difficult.
This may include adults who:
- Have significant mobility limitations
- Use a walker, wheelchair, or other assistive device
- Are medically frail
- Manage several chronic conditions
- Have recently returned home after hospitalization or rehabilitation
- Have substantial difficulty arranging transportation
- Need caregiver support during medical appointments
You do not need to determine whether you qualify before contacting Renovare.
Our team can review the patient’s location, general medical needs, and requested service to determine an appropriate next step.
How Renovare Evaluates Your Needs
The purpose of a home visit is similar to an office-based primary-care appointment, but the assessment takes place in the patient’s residence when that setting is clinically appropriate.
Your provider may review:
- Current symptoms and health concerns
- Medical conditions
- Current medications
- Recent hospital or specialist care
- Home blood-pressure or glucose readings
- Changes in mobility or functioning
- Caregiver concerns
- Follow-up and preventive-care needs
Not every medical concern can be fully evaluated in the home.
Renovare may recommend laboratory testing, imaging, specialist care, an office appointment, urgent care, or another setting when additional evaluation is needed.
How it works
Primary care home visits may include
Medical assessment & follow-up
Evaluation of appropriate symptoms, stable chronic conditions, home measurements, and changes in health.
Medication & post-hospital review
Review medications, discharge instructions, recent treatment changes, and follow-up needs after hospitalization.
Referrals & care coordination
Testing, specialist referrals, therapy, home health, or other services may be coordinated when appropriate.
Medication and chronic-care follow-up
A home visit can be useful when a patient takes several medications or needs follow-up for stable chronic medical conditions within Renovare’s scope.
Your provider may review current prescriptions, over-the-counter products, supplements, home measurements, recent laboratory results, medication response, and possible side effects.
Patients should have an accurate medication list or medication containers available whenever possible.
Never stop or substantially change a prescription without discussing it with the treating clinician.
Care after hospitalization or rehabilitation
Returning home after a hospital or rehabilitation stay can involve new medications, new diagnoses, and several follow-up instructions.
A Renovare home visit may help review:
- Current symptoms
- Medication changes
- Discharge instructions
- Follow-up appointments
- Testing or specialist recommendations
- Mobility or caregiver concerns
Bring discharge paperwork and an updated medication list whenever possible.
Rapidly worsening or emergency symptoms should not wait for a scheduled home visit.

When Immediate Care Is Needed
Renovare provides scheduled outpatient medical care, not emergency house calls. Call 911 or go to the nearest emergency department for severe chest pain, severe breathing difficulty, signs of stroke, loss of consciousness, serious injury, uncontrolled bleeding, sudden severe confusion, or another medical emergency.
Request a primary care home visit
If traveling to a medical office is difficult because of health or mobility concerns, Renovare can help determine whether a home visit may be appropriate.
Submitting a request does not establish a patient-provider relationship or guarantee home-visit eligibility, treatment, referral, or insurance coverage.
Not an emergency service. For an immediate medical or psychiatric emergency, call 911 or go to the nearest emergency department.
